Fatal accident victim identified

Authorities identified Oakdale resident Richard G. Haynes as the man who was run over yesterday by a dump truck near the Calaveras-Tuolumne county line.

Haynes, 39, was coupling a trailer to a dump truck on the shoulder of O'Byrnes Ferry Road about 7:45 a.m. when the fully loaded truck rolled backward, said California Highway Patrol Officer Bryan Duquesnel.

The left side of the truck ran over Haynes, who died at the scene, according to a CHP report.

Investigators suspect the truck's brakes were released, either because of human error or mechanical failure, Duquesnel said, but the CHP is still investigating.

"We are trying to get the answers, and we have specialists coming down to do a complete inspection on the truck," he said.

The truck, now at the CHP office in San Andreas awaiting a Monday inspection, belongs to Black Diamond, a Modesto gravel hauling company.

Haynes was picking up the trailer because it had been left a day earlier near the bridge over Tulloch Reservoir because of a mechanical failure.

The owner of Black Diamond was at the scene at the time of the accident and called 911 on his cellular phone, Duquesnel said.

The accident blocked one lane of O'Byrnes Ferry Road, and traffic was directed around the scene.
